While trying to get her life back on track, Esme's biological father arrives and is in desperate need of a transplant. Catherine is faced with the difficult decision of having to reveal the truth to Esme about who her real parents are. Owen is agreeable to having Catherine back at work, though it isn't without strict parameters, making it a challenge for Catherine to earn her credibility back.

Catherine is experiencing hallucinations and refuses Dr. Hartramph's urging to be admitted into a hospital. Her behavior is becoming increasingly manic and it isn't long before Mahmoud begins to piece together what she's seeing. Meanwhile, Bickman is reluctant to go back into the operating room after Joey's botched surgery. Catherine encourages Bickman to see Lina so that she can re-wire his brain. A woman confronts a knife-wielding man on the street and the team wonders if it's really a display of bravery or if she has an absence of fear.

Dr. Bickman rushes his doorman to The Cube after he sees him collapse from a buried aneurysm. With a second bleed imminent, he makes a choice to perform an alternative procedure to repair it. After revealing a difficult diagnosis to a young mother, Catherine finds herself sinking into a deep sadness. In her desperation, she misleads a nurse into supplying her with oxycodone. Catherine turns to her brother for help, though he begins to wonder if he’s become an enabler. Catherine reveals to Will that their relationship is over.

Dr. Morely admits a patient who is convinced he is dead. Doomed to enduring the advanced onset of the hallucination, he reluctantly agrees to some tests that reveal he has Cotard’s Syndrome. A mother is at the end of her rope when her daughter displays signs of meningitis and fails to be diagnosed by several other doctors. Esme is hired at Will’s restaurant to help out, and it isn't long before she discovers the truth about Delilah. Will confronts Catherine when he thinks she is hiding a secret. In a panic, Catherine reveals the truth only, not the one he was looking for.

Catherine meets Will’s parents and begins to panic at the thought of turning into a boring housewife.

To her surprise, she begins to see vibrant visions of snow leopards and landscapes that she could have never imagined. Elsewhere, Will discovers that he can't get rid of Delilah without risk of losing his restaurant.

Catherine and Owen use unconventional methods to help Dr. Reynaud deal with his crippling fear of death so that he can be at peace in his final days.

swf64 Enabling failure and manipulation are the hallmarks of the American entertainment industry. For the most part it's a harmless game. It always has been.It's not wrong when the ends justify the means, or there is another justifiable framework in place. Wrong becomes right and right becomes wrong. You're not to blame and the enemy is always a bad guy. It's a fantasy that sells. It's a fantasy that entertains.Sex, violence, power, all looking for justification and an audience to entangle. The only goal to sell and support an above average lifestyle. These are the hard truths and you will find nothing like them in the Black Box.These are the reasons some topics are generally seen as untouchable except by those with no scruples.If ABC was really interested in helping the mentally ill navigate our society, I would suggest a television series might not be the best vehicle.
andreousoulla I absolutely totally and utterly love this show and found myself hooked from the very start. Dr Black is a natural and i love how she can quickly see the signs and diagnose a patient. The treatment she comes up with is sometimes questionable but as in the real world, if it works for the client then it will do. I too work with people who have disabilities and so far the show is accurate and although dramatic paints a very interesting and enlightening picture of how the minds of these people register and react. i also love the way they are portrayed and that sometimes we may see a mental illness in a negative way but if seen from another perspective, it is comforting. please keep this show going! Ratings alone shouldn't determine whether the show continues!
bsquared-23-737287 Yes, one of the best programs I have watched in several years. I am a psychiatric social worker, and I case manage the chronically mentally ill weekly, and have for many years. I also have a strong background as a psychiatric-hospital social worker, and have worked with some of the best and progressive Psychiatrists in Southern California. so, when I say "Wow",you can bet that this program is a real touch of reality, and way to late to ell the story of what it is to be genetically insane. Recentevents have given this program real substance. At 72 years old I only wish I could be involved in this program. You guys are special. Please,keep up the good work. And keep us guessing. chez-mays I have become addicted to this TV show, it is a great connector for me. I have Bipolar Disorder and so many things in this show are how I lived off medication, on medication or screwing around with medication (because reality is medication is very hard core and not nice to take) The world on Mania IS exactly how it is perceived in this show, you want to literally fXck anything that moves, hallucinate and go a bit "crazy". And you're reckless (hence where she was dancing on the balcony) We tend to screw up relationships because sometimes we just become "numb" and loose all emotions of actual care to make it work because we get to a thought process of "why should I, I'm just insane and they wont love me"Really enjoy this show, it's a very deep reality into mental illness and that might not be everyone's cup of tea but it's a living reality for many of us...and yes a lot of mentally ill people watch TV (referring to a comment made by another use that we apparently don't watch TV)
